I once had to proofread a translation made by a department of a large company for their top management. The text featured the term “flat rate”. One of the meanings the word “flat” has in English is that describing a residential unit in a multi-storey building. The company's employee that did the translation thought that this was the meaning implied, and “flat rate” turned into a “utility bill” in Russian.
The text was about a fixed fee though, so the meaning of the translation was twisted beyond recognition. Good thing we noticed this in time to fix the mistake!
